(Contract Position until March 31, 2018, with possibility of extension to March 31, 2019)
Stella’s Circle and Choices for Youth are currently looking for a Case Manager to fill a position on an Intensive Case Management (ICM) and Housing Supports program. The City of St. John’s is partnering with End Homelessness St. John’s (EHSJ) to deliver this program for chronically and episodically homeless persons in St. John’s.
The Front Step program is shared between both organizations as we work to evolve the program by developing, implementing and providing day to day leadership and support. This program is expected to run until March 2018 with a possibility of renewal.
St. John’s Community Plan to End Homelessness has proposed a vision to end homelessness in St. John’s by 2019. The plan is based on a systems approach grounded in Housing First as the guiding philosophy. The plan sets out 4 priority areas which will result in further development of specific aspects of the continuum of housing and related supports for individuals who experience homelessness. These include systems coordination; integrated information system and research; leadership and resources; and housing supports.
As Case Manager, you will provide individualized support services to clients in order to promote recovery, stability and independent living. You will use a strengths-based approach and will be required to have flexibility to hours of work and a willingness to be creative in motivating and inspiring clients.
Duties include but are not limited to:
Provide intensive case management to program clients;
Provide ongoing emotional support, encouragement and unconditional acceptance to clients;
Identification of client needs and development of an individualized service plan in conjunction with the client and team;
Conduct risk assessments and establish safety plans with clients;
Assist clients in all aspects of housing – searching, moving, developing landlord-tenant agreements, financial resources, etc;
Help the client recognize, develop and utilize the support networks in his/her life;
Initiation of appropriate community referrals and/or collaboration with other as required on behalf of clients;
Focus on crisis-prevention through problem-solving and planning with clients;
Manage crisis situations which do occur and implementation of appropriate action plans;
Implement an Eviction Prevention Program – providing direct support to tenants on housing retention, mediation and conflict resolution;
Seek out and develop private landlord market for potential rentals;
Ensure all documentation, case management reports and outcome measures for clients are current and complete.
The successful candidate will:
Have a post-secondary degree in a social sciences field such as social work, psychology, etc; Equivalent combination of education, training and experience will be considered;
Must be eligible for registration with respective designation (ie: nursing, psychology, social work);
Have a minimum of 3 years’ direct work experience with target population being served in program;
Have a knowledge, understanding and commitment to the Housing First Model, Recovery Model and philosophies;
Have extensive knowledge and experience in such areas as housing, homelessness, mental health, addictions, trauma, conflict resolution and crisis intervention;
Have experience in partnership-based programs and services with excellent knowledge of resources available to our target population.
Excellent clinical, analytical and problem solving skills;
Demonstrated documentation, computer, time management, communication, organizational and interpersonal skills with an attention to detail;
Must have a valid driver’s license and access to a safe vehicle for work;
Must have a valid First Aid/CPR Certificate and ASIST training.
The annual salary is $60,000 which is based on a 37.5 hour work week. This position comes with a benefits package including health, dental, life insurance, ADD, LTD and an RRSP plan.
